Every year a significant number of hospital admissions and deaths occur through
poor medication use. Government and medical bodies are continually trying to
prevent this from occurring. Current strategies include targeting self efficacy
and education. These strategies are normally carried out by consultant pharmacists
who visit clients at home and sit together to review their current medications,
how to take them and remove any out of date medications.
The normal process involves a GP identifying ‘at risk’ patients,
the patient agreeing and nominating their Community Pharmacy and this pharmacy
using them or finding a consultant pharmacist to conduct the review and reporting
back to the GP their findings. A management plan is then formulated with the
GP and patient. The process of ordering / reporting is inconsistent and sometimes
critical information is missed. This system looks to address this through the
use of software and hardware.

- information is synchronised with eeePc (linux system) – recommend Java
- a standard set of information items are completed including taking any imagery
- reports are submitted back through the portal to the GP or Pharmacist
- GP/pharmacist receive the report through the portal and download it into their
clinical system
System will require:
- secure web portal
- sms gateway integration
- java database and UI development
- integration with Linux OS
